Once you decided to start our program, here are the steps to follow:
Use this link “Start Registration” to schedule a registration only appointment (not an actual appointment). You’ll receive another link by text message that will take you to the registration forms. Please do not skip any sections, including : Patient information, insurance card & ID (front and back), clinical history questionnaire (must be filled out completely), pharmacy info and Consent forms.
- Office services: We no longer accept the following insurances: any HMO plans, Cigna, United healthcare, UMR, Tricare and Medicaid/Medicare. Please check if we are in-network with your insurance plan. If not, we offer a self pay package. Please contact Lisa for more information.
- Compassionate care program is available for qualified patients. Please contact Lisa for more information.
- Lab services: The office will provide a list of CPT codes for the labs ordered. Please check lab coverage with your insurance prior to the first visit.
- Once your portal account is activated, please contact your local OBGYN and Reproductive endocrinologist, and request a referral and medical records to be sent to us via fax 833-818-0124. We no longer can receive medical records via email. This is important prior to first visit.
- Out of state patients are required to be seen in the office at least the first visit, then subsequent visits can be provided through telemedicine (check with insurance if telemedicine is a covered service).
